If your account has metered service, please call AWWU Customer Service at 564-2700 to schedule a final meter read and to discontinue service.
Do not submit this form if you have a metered service.
Request for disconnects can only be made with a future date.
If a deposit was required on your account, it will be credited to your final bill. If service at another location within the AWWU service area is needed please complete the APPLICATION FOR SERVICE request.
If known, please provide the name and daytime telephone number of the individual that will be taking over the service at this location.
After the application is submitted, you will receive a confirmation. If you do not receive a confirmation, that means an error has occurred. Please report the error to AWWU Customer Service at 564-2700.
