2008 Benefit Enrollment

You may change your benefits during the year if you meet the listed criteria below:

  • Have a Family Status Change (FSC) such as the birth of a child, marriage, divorce, death of a spouse, etc., and you work a minimum of 20 hours per week.

  • Accepted a job in a different labor group, such as a non-rep who accepts an AMEA position, and you work a minimum of 20 hours per week.

If you do not meet one of the above-mentioned criteria then you cannot change your benefits until the next  Benefit Open Enrollment period which will be held from November, 2008 through December, 2008.

For a Family Status Change, you have exactly 90 days to turn in your proof of FSC, such as a marriage certificate, divorce decree etc.  You may turn in a hospital record of birth until the state birth certificate can be obtained.

All enrollment forms are PDF/Fillable. Each enrollment form can be completed online. Print, sign and return the form to AWWU Employee Services within 30 days of your family status change or labor group change.

Coverage for your new added family member will be effective on the date of birth/adoption or marriage.

Coverage for a labor group change will be effective on the first day of the next month.
